New Patients

Guide to your first appointment:

We schedule a brief introductory phone call for new prospective patients. We do not charge for this
introductory call. This is an opportunity to meet Kywina Henderson, Psych NP, the clinician, by phone and
see if there is good potential for a therapeutic and productive relationship. It is also an opportunity to
review practice clinical policies and answer questions. The clinical inquiry phone call is confidential and no
patient record will be created. If we agree to move forward, you will receive a call back for the next
available intake appointment.

Appointments and Scheduling

All appointments are held via a secure telemedicine system that you can access via a desktop computer, a
tablet or mobile device connected to the Internet. You must have an Internet connection capable of
streaming video to have a satisfactory telemedicine session. A secure link is provided in your appointment
confirmation email or text message. You will receive an appointment reminder. Please make sure you are
in a comfortable, private area and are in your home state at the time of the appointment. For your safety,
please make sure you are not driving during your appointments.

Before Your First Appointment

We are a telemedicine practice so we collect all information needed online. There will be several forms
required to verify your identity, obtain payment, discuss consents for care and detail your relevant medical
history.

**You will be required to create a “New Sign Up” allowing you to create a password and sign into the
confidential patient portal hosted through our partner Optimantra.

The patient portal is how we communicate privately with our patients. We use this to obtain and send you
medical records, educational materials and communicate with you about treatment, labs, prescriptions and
etc.

Identifying information

We will need an email address, phone number and your full name & date of birth to communicate. We will
also require a valid driver license or government-issued photo identification to confirm the identity of new
patients. *You will need to upload this into the documents section of your portal under the appropriate
section (ID Card)

Payment for Telehealth Service

We provide a secure online portal for payment. Payment is expected at time of service and this needs to be
added to your patient portal profile.

Health Insurance Verification

Please note that if you wish to use health insurance, the coverage must be verified before we will confirm
your appointment. To allow for review of the information you submitted, we require patients to complete the
intake forms at least 3-business days before their scheduled first appointment. Please check your plan for
coverage and make sure virtual care from your provider network is a benefit. **When creating an online
portal account and completing forms, please include the front and back of your insurance card in the
documents section so we can contact your plan administrator to verify coverage.
